Supply Care Solutions is currently recruiting on behalf of Walsall Council for an experienced Social Care Facilitator to join their Adult Social Care Team.
This is an excellent opportunity to make a real difference in the lives of vulnerable adults by delivering person-centred assessments, care management and support planning within a collaborative multidisciplinary team. You’ll play a key role in promoting independence, wellbeing and positive outcomes for adults across the borough.
🌟 Key Responsibilities
✅ Assess, plan, coordinate and review care and support for vulnerable adults
✅ Develop person-centred support plans that promote independence and choice
✅ Undertake strengths-based assessments for adults and their carers
✅ Manage a varied caseload, ensuring interventions are outcome-focused and responsive
✅ Promote health, wellbeing, independence and safeguarding in line with current legislation
✅ Apply the principles of the Mental Capacity Act and self-directed support
✅ Build positive relationships with service users, families, carers and partner agencies
✅ Work collaboratively with multidisciplinary teams to deliver coordinated care
✅ Identify simple assistive equipment needs and arrange provision where appropriate
✅ Refer complex equipment and adaptation requirements to specialist services
✅ Maintain accurate, confidential case records and professional documentation
✅ Participate in supervision, training and ongoing professional development
✅ Requirements
✔ Previous experience within Adult Social Care or a similar statutory setting
✔ Experience carrying out assessments, care planning and care management for vulnerable adults
✔ Good knowledge of Adult Social Care legislation, safeguarding and person-centred practice
✔ Understanding of strengths-based approaches and self-directed support
✔ Excellent communication and relationship-building skills
✔ Strong organisational skills with the ability to manage a varied caseload
✔ Experience working effectively within multidisciplinary teams and alongside partner agencies
✔ Competent IT skills and experience maintaining accurate electronic case records
✔ Proactive, flexible and solution-focused approach to supporting vulnerable adults
✔ Full UK Driving Licence (Essential)
