About the Role:

We’re currently recruiting for an experienced and qualified Adults Social Worker to join Halton Council’s Complex Care Team.

The successful candidate will manage a complex caseload, undertaking assessments, reviews and care planning while working collaboratively with individuals, families, carers and multi-disciplinary professionals to achieve positive outcomes.

This is an excellent opportunity for an experienced Social Worker who is passionate about person-centred, strengths-based and outcome-focused practice and is looking to make a positive difference within Adult Social Care.

Key Responsibilities:

• Manage a complex caseload in line with relevant legislation, statutory duties and local policies
• Complete comprehensive assessments, reviews and care planning
• Safeguard and promote the wellbeing, independence, dignity and rights of adults
• Work collaboratively with individuals, families, carers and partner agencies
• Develop, implement and regularly review appropriate care and support plans
• Identify and respond appropriately to safeguarding concerns and risks
• Support individuals to maximise their independence and achieve positive outcomes
• Maintain accurate, timely and up-to-date case records and documentation
• Contribute effectively to multi-disciplinary working and professional discussions
• Liaise with health professionals and other relevant agencies to coordinate effective support
• Ensure all practice is person-centred, strengths-based and outcome-focused
• Apply relevant legislation, policies and professional standards to social work practice
• Complete clear and accurate reports and professional documentation

Requirements:

• Recognised Social Work qualification
• Current Social Work England (SWE) registration
• Previous experience working within Adult Social Care
• Experience managing complex cases and caseloads
• Strong knowledge of relevant adult social care legislation and statutory responsibilities
• Good understanding of safeguarding, assessment and care planning processes
• Excellent communication, assessment and report-writing skills
• Ability to build effective professional relationships with individuals, families and partner agencies
• Ability to work independently and manage a complex workload effectively
• Ability to work collaboratively within a multi-disciplinary team
• Strong organisational, analytical and decision-making skills
