About the Role:
We are currently recruiting on behalf of our client for an experienced Locum Social Worker to join the Willow Therapy Unit based at Norwich Community Hospital. This is a rewarding opportunity to work within a supportive multidisciplinary team, helping adults achieve safe and timely hospital discharges while promoting independence, recovery, and wellbeing.

Working within a hospital setting, you will collaborate closely with health and social care professionals to deliver high-quality, person-centred support. The service operates under a Home First ethos, focusing on recovery, independence, and partnership working.

You will also support individuals following discharge, carrying out reviews in their own homes or within care settings to ensure appropriate ongoing support is in place.

Key Responsibilities:
• Undertake person-centred assessments with patients and their families
• Develop and implement care plans that promote independence and wellbeing
• Work collaboratively with multidisciplinary teams to facilitate effective discharge planning
• Conduct post-discharge reviews and assessments
• Support service users in achieving the best possible outcomes through strengths-based practice
• Complete Mental Capacity Assessments and Best Interest decisions where required
• Maintain accurate case records and contribute to service development

Requirements:
• A recognised Social Work qualification and current Social Work England registration
• A sound understanding of the Care Act 2014 and its practical application
• Experience completing Mental Capacity Assessments and Best Interest decisions
• Knowledge and experience of Continuing Healthcare (CHC) reviews
• Strong communication, organisational, and case management skills
• The ability to work effectively in a fast-paced hospital environment
• Access to a vehicle and willingness to travel as required
• Previous experience within adult social care and hospital discharge services is desirable
