About the Role

Supply Care Solutions is currently recruiting for an experienced Personal Advisor to join Shropshire Council’s Leaving Care Team.

This is an excellent opportunity for a dedicated professional who is passionate about supporting young people and care leavers to achieve positive outcomes as they transition into adulthood and greater independence.

You will manage a complex and diverse caseload, including former and current asylum-seeking children and young people, those with unresolved immigration status and young people with disabilities.

You will undertake needs assessments, develop appropriate plans and work alongside young people and partner agencies to promote independence, stability and positive outcomes.

Key Responsibilities

As a Personal Advisor, your responsibilities will include:

  • Managing a complex and diverse caseload of young people and care leavers.
  • Working directly with service users to identify and achieve their goals.
  • Undertaking needs assessments and developing plans that respond to individual needs.
  • Producing and reviewing Pathway Plans in line with statutory guidance and local procedures.
  • Ensuring Pathway Plans are SMART, outcome-focused and centred on independence and successful transition to adulthood.
  • Working collaboratively with a range of agencies and professionals involved in care planning.
  • Promoting positive outcomes and ensuring services respond effectively to the needs of young people.
  • Building enduring support networks for care leavers based on trust and confidence.
  • Understanding and responding appropriately to the individual needs, circumstances and diversity of each care leaver.
  • Encouraging care leavers to participate in decision-making and contribute to the development of the Leaving Care Service.
  • Working independently with care leavers and their families.
  • Producing clear, concise and accurate reports.
  • Using supervision, professional discussions and appropriate assessment tools to support effective decision-making.
  • Taking responsibility for your own professional development, boundaries and relevant training.
  • Working in accordance with relevant legislation, statutory guidance, local procedures and professional standards.

Requirements

The successful candidate will ideally have:

  • Previous experience working with care leavers, young people or vulnerable young people.
  • Experience managing a complex and diverse caseload.
  • Strong knowledge of Pathway Plans, needs assessments and transition to adulthood.
  • Experience working with multi-agency professionals and partner organisations.
  • An understanding of the challenges faced by care leavers, including those with immigration, disability or additional support needs.
  • Strong communication, assessment and report-writing skills.
  • The ability to work independently while contributing effectively to a wider team.
  • A commitment to promoting independence, participation and positive outcomes for young people.
  • A valid Enhanced DBS with Adults & Children’s Barred List.
  • A full UK driving licence and ability to travel as required.

Working Arrangements

The role operates Monday to Friday:

  • Monday–Thursday: 8:45am–5:00pm
  • Friday: 8:45am–4:00pm

The assignment is initially for 12 weeks, with the possibility of extension.
