About the Role
Supply Care Solutions is currently recruiting for an experienced Adults Social Worker to join Peterborough City Council’s Hospital Team, based at Peterborough City Hospital.
This is an excellent opportunity for a qualified Social Worker with strong experience in Adults Social Care, hospital discharge, assessment, safeguarding and care planning.
The successful candidate will assess the needs of adults who potentially or currently meet the eligibility criteria for services, formulate appropriate care and support plans, and monitor and review services to ensure individuals’ needs are being effectively met.
You will manage a varied and complex caseload, including adult safeguarding matters, while working collaboratively with service users, carers and other professionals.
A key focus of the role will be supporting individuals who may be at increased risk of hospitalisation or admission to long-term care, while promoting choice, independence and involvement in decisions about their care.
Key Responsibilities
As an Adults Social Worker, your responsibilities will include:
- Undertaking timely and holistic assessments of individuals and, where appropriate, their carers.
- Assessing eligibility for services in accordance with relevant legislation, policies and best practice.
- Managing a varied and complex adult social care caseload.
- Undertaking adult safeguarding investigations and implementing appropriate protection plans.
- Formulating, implementing and reviewing care and support plans.
- Promoting choice, control and independence in care planning and decision-making.
- Monitoring the suitability and effectiveness of services against identified needs.
- Working within the relevant legal framework when assessing mental capacity.
- Supporting individuals who lack capacity to make specific decisions.
- Advising individuals and carers about services available through other organisations.
- Supporting applications, referrals and access to appropriate services.
- Maintaining accurate, comprehensive and up-to-date case records.
- Working collaboratively with colleagues across health and social care services.
- Developing effective partnership working with external agencies and professionals.
- Supporting the ongoing development and improvement of services within the team.
- Supporting the professional development of students, care support workers and other relevant team members where required.
- Contributing to supervision and Personal Development Plans where appropriate.
- Maintaining your own continuing professional development.
- Supporting the Team Manager with quality assurance systems and ensuring the team’s responsibilities are delivered consistently and to the required standard.

Requirements
The successful candidate will ideally have:
- A recognised Social Work qualification.
- Current registration with Social Work England.
- Experience working within Adults Social Care.
- Experience working within a hospital or discharge setting is highly desirable.
- Strong experience undertaking adult social care assessments.
- Experience managing complex caseloads.
- Experience undertaking adult safeguarding investigations.
- Knowledge and understanding of Mental Capacity Act principles.
- Experience developing, implementing and reviewing care and support plans.
- Strong understanding of relevant adult social care legislation and statutory guidance.
- Excellent assessment, analytical, recording and report-writing skills.
- Strong communication and partnership-working skills.
- Ability to work effectively with service users, carers and multi-disciplinary professionals.
- Commitment to promoting choice, independence and person-centred care.
- Ability to work independently while contributing positively to a wider team.
Working Arrangements
The role is based at Peterborough City Hospital and requires 37 hours per week, Monday to Friday.
Approximately every 11β12 weeks, the successful candidate will be required to work a 7-day period, including the weekend. Monday and Tuesday will then be provided as days off in lieu.
