About the Role:
We are currently recruiting on behalf of Somerset Council for an experienced Adult Social Worker to join their Learning Disabilities Review Project.
This is an excellent opportunity for a practitioner with strong Care Act knowledge and significant Learning Disabilities experience to undertake detailed reviews and reassessments, ensuring individuals receive appropriate, person-centred, and evidence-based care and support.
The successful candidate will work autonomously while collaborating closely with health professionals, care providers, families, advocates, and partner agencies to deliver high-quality assessments and defensible professional decision-making.
Key Responsibilities:
β’ Undertake detailed Care Act reviews and reassessments for adults with learning disabilities.
β’ Conduct face-to-face visits within individuals’ homes, supported living services, residential settings, and community environments.
β’ Gather and analyse information from individuals, families, providers, advocates, health colleagues, records, and direct observations.
β’ Determine whether a statutory review or full reassessment is required and provide a clear professional rationale.
β’ Identify and evidence eligible care and support needs in line with the Care Act.
β’ Produce high-quality, evidence-based, audit-ready assessments, reviews, support plans, and case recordings.
β’ Support and contribute to CHC processes, funding reviews, and multidisciplinary discussions where required.
β’ Work collaboratively with Adult Social Care teams, health colleagues, providers, and stakeholders to achieve timely outcomes.
β’ Maintain accurate electronic records and ensure compliance with relevant legislation, guidance, and best practice.
β’ Manage an independent caseload effectively and deliver reviews within agreed project timescales.
Requirements:
β’ Current Social Work England registration.
β’ Significant post-qualifying Adult Social Care experience.
β’ Strong experience working with adults with Learning Disabilities.
β’ Experience completing Care Act reviews and reassessments.
β’ Excellent assessment, analytical, and evidence-based decision-making skills.
β’ Ability to identify eligible care and support needs under the Care Act.
β’ Experience working within multidisciplinary teams and partnership settings.
β’ Knowledge of CHC processes, including Checklists, DSTs, joint funding arrangements, and health and social care boundaries.
β’ Experience challenging funding decisions through professional evidence and clear rationale.
β’ Ability to work autonomously, manage workload effectively, and deliver within project timescales.
β’ Strong IT and electronic case recording skills.
β’ Full UK Driving Licence and access to own vehicle.
β’ Ability to travel throughout Somerset and attend the office at least two days per week.
Desirable Experience:
β’ Experience working with adults with Learning Disabilities across a variety of settings.
β’ Previous supervisory experience.
β’ Understanding of social care resources beyond statutory services.
β’ Awareness of current legislation, government guidance, and developments affecting Adult Social Care.
