💰 Rate/Salary: £16 – £23 per hour (depending on skills and experience)

About the Role:
We are currently recruiting for a Through Care Navigator to join the Rough Sleeping Team in Slough. This is a vital and rewarding position supporting some of the borough’s most vulnerable individuals.

You will work as part of a multi-agency team to reduce and prevent rough sleeping by delivering tailored, person-centred support to those currently sleeping rough or at risk of homelessness. This role focuses on helping individuals access housing, healthcare, substance misuse services, and training opportunities, enabling them to move towards a more stable and independent life.

Key Responsibilities:
• Build and maintain effective relationships with housing providers, healthcare professionals, and local authorities
• Provide holistic support focusing on accommodation, health, wellbeing, and employment
• Develop and manage individualised support plans for clients, identifying and mitigating tenancy risks
• Collaborate with Outreach, Housing Demand, and Temporary Accommodation teams to identify those at risk of homelessness
• Conduct client visits at temporary placements, rough sleeping sites, or supported housing schemes
• Participate in early morning or evening outreach sessions as required
• Maintain accurate case notes, assessments, and support plans
• Contribute to local rough sleeper counts and provide flexible cover for colleagues when needed

Requirements:
• Experience supporting vulnerable adults affected by homelessness, mental health, or substance misuse
• Strong understanding of housing and homelessness services, legislation, and multi-agency collaboration
• Excellent communication and interpersonal skills
• Proven ability to manage a varied caseload effectively
• Flexibility to work occasional unsocial hours (evenings, early mornings)
• Enhanced DBS (or willingness to obtain one)
• UK driving licence and access to a vehicle desirable but not essential
