Supply Care Solutions are looking for a Social worker within Adults Mental Health Team. in the East Midlands

SALARY: £31.00 dependant on experience
TYPE: LOCUM
TEAM: Adult Mental Health team
HOURS: This is a Full time role – Mon-Thurs 8.30 – 5.00 Friday 8.30 – 4.30
Working Pattern: Flexibility around the needs of the service but will need to be in the office at least 3 days and will also have to make hospital visits

This remit also includes service users who have a diagnosis of mental illness and Autism/ mental illness.
To complete assessments, to supervise care and support plans, advice on complex cases, risk manage.
To case manage complex cases, complete care and support assessments/plans, reviews and commission safe cost effective packages of care and placements usually via Direct payments.
To attend the multi-disciplinary team meeting to promote best practice and joint working.
To fulfil the statutory requirements of RCC under the Care Act 2014.
To undertake safeguarding enquiries and convene multi agency meetings around vulnerable adult risk management meetings
To work with our legal department on cases where ordinary residence and Sec117 aftercare entitlement are in dispute.
To complete reports for Mental Health Tribunals and managers hearings and participate in safe discharge planning. This will involve visiting inpatient sites.
Support with adult duty and add to the capacity of adult social care.

Qualified Social Worker Requirements

– CSW/CQSW/DipSW/BA/MA in Social Work
– Proven post qualifying experience in working with Adult services
– Current Social Worker England registration
– Current Enhanced DBS Disclosure
– Eligibility to work in the UK without work permit or visa restrictions
– Commitment to the safe care and well-being of vulnerable people
