• Social Worker provides case management and supportive services to children with disabilities and their families. The goal is to promote the well-being, safety, and development of children with physical, intellectual, or developmental disabilities by connecting them to necessary supports and ensuring access to services that enhance their quality of life and family stability.
• Conduct comprehensive assessments of children with disabilities and their families to determine service needs and eligibility.
• Develop and maintain individualized case plans that address each child’s unique developmental, medical, educational, and emotional needs.
• Coordinate and monitor services such as in-home care, respite, therapy, educational support, and medical resources.
• Advocate for children’s rights and access to services within schools, healthcare systems, and community agencies.
• Support families in understanding and navigating complex systems (e.g., Regional Centers, IEPs, IHSS, SSI).
• Collaborate with interdisciplinary teams, including educators, medical professionals, therapists, and service coordinators.
• Maintain timely and accurate documentation in compliance with agency policies, county/state regulations, and legal standards. 
• Conduct home visits, safety checks, and participate in team or interagency meetings. 
• Respond to concerns regarding child safety and well-being, making referrals or reports as necessary.

3 years post qualifying experience plus ASYE required.
