About the Role
We’re recruiting for a Social Care Practitioner / Community Care Worker to join our client’s North Occupational Therapy Team. This is an excellent opportunity for a Community Care Worker or Social Care Practitioner with strong assessment skills to support a focused Occupational Therapy project running until March 2026.

You’ll play a key role in addressing a backlog of non-complex Occupational Therapy assessments, helping people remain independent in their own homes through equipment provision, and minor and major adaptations. Working as part of a supportive and experienced Adult Care team, you’ll collaborate closely with Occupational Therapists, Social Care Practitioners, Housing and Health teams to deliver proportionate, person-centred support.

Key Responsibilities
• Conduct strengths-based assessments of individuals’ needs and daily living activities
• Work with individuals, families and partner agencies to promote independence and well-being
• Liaise with equipment providers and housing teams to implement practical solutions
• Maintain accurate and timely case records (experience using Mosaic desirable)
• Apply safeguarding principles and relevant legislation in everyday practice
• Participate in supervision, reflective practice, and professional development

Requirements
• Level 3 Diploma in Adult Care (or equivalent/higher qualification)
• Proven experience assessing needs within adult social care or a local authority
• Strong interpersonal and communication skills with a partnership approach
• Good IT and time management skills
• Understanding of equipment and adaptation solutions, and their role in supporting independence
• Ability to manage a caseload effectively and travel for home visits
