Liverpool city council is dedicated to enhancing the well-being of individuals within our community. We are seeking a compassionate and skilled Social Care Assessor to join our team in Liverpool, contributing to the delivery of high-quality social care services.

Key Responsibilities:

Conduct Assessments: Perform comprehensive assessments of individuals’ social care needs, including Reablement and Care Act assessments, to determine eligibility and develop tailored support plans.

Develop Support Plans: Collaborate with service users and their families to create detailed, person-centered support plans that promote independence and well-being.

Implement and Monitor Services: Coordinate the delivery of agreed-upon services, ensuring they meet the identified needs and adjusting plans as necessary based on ongoing evaluations.

Provide Information and Advice: Offer guidance to individuals and carers on accessing universal services and support, promoting self-directed support and informed decision-making.

Collaborate with Professionals: Work alongside healthcare providers, social workers, and other relevant professionals to ensure a holistic approach to care. 

Maintain Accurate Records: Document assessments, plans, and any changes accurately and promptly, adhering to organizational policies and data protection regulations.

Qualifications and Experience:

Educational Background: A relevant qualification in social care or a related field is preferred.

Experience: Proven experience in social care assessment, support planning, or a similar role is advantageous.

Knowledge: Understanding of the Care Act, Mental Capacity Act, and other relevant legislation and policies.

Skills: Excellent communication, interpersonal, and organizational skills, with the ability to work independently and as part of a team.
