About the Role:
Supply Care Solutions is urgently recruiting an experienced Senior Social Worker or Advanced Practitioner to join Telford & Wrekin Council’s Adults TICAT team. This role sits within the Intermediate Care and Assessment Service, supporting adults in need of timely assessments, interventions, and safe discharge planning from the Princess Royal Hospital. You will work in a fast-paced, multidisciplinary environment and play a key role in preventing discharge delays, supporting strengths-based practice, and promoting safe transitions back into the community.

Key Responsibilities:
• Complete Care Act 2014 assessments in hospital and community settings
• Facilitate safe, timely hospital discharges through coordinated planning
• Manage a varied and complex caseload with professional autonomy
• Lead safeguarding enquiries under Section 42
• Conduct Mental Capacity Act assessments and best interest decisions
• Provide expert guidance to colleagues and partner professionals
• Collaborate with health teams (OTs, physios, nurses, ward staff)
• Develop proportionate, person-centred support plans
• Maintain accurate case notes in line with local authority standards
• Identify and manage risk effectively, escalating concerns appropriately
• Contribute to MDTs, ward rounds, huddles, and case discussions
• Build strong relationships with service users, carers, and external agencies
• Promote independence, prevention, and reablement wherever possible

Requirements:
• Qualified Social Worker (BA/MA Social Work or equivalent)
• Social Work England registration
• Proven experience in adults’ statutory services, ideally hospital discharge, rapid response, or intermediate care
• Strong knowledge of the Care Act, MCA, safeguarding, and relevant legislation
• Enhanced DBS on the Update Service
• Ability to make sound, defensible decisions under pressure
• Strong IT and case-recording skills
• Flexibility to work across hospital, community, and home-based settings
