Job Overview:
Walsall Council is seeking a qualified and experienced Senior Social Worker to join a Multi-Disciplinary Team (MDT) in supporting individuals following hospital discharge. As part of the Adult Social Care team, you will be responsible for reviewing and assessing packages of care at home and short-term placements, working towards promoting independence and ensuring long-term care needs are met where appropriate.

Key Responsibilities:

• Work within an MDT team to complete initial assessments and subsequent reviews of home care packages and short-term placements after hospital discharge.
• Identify whether Occupational Therapy (OT) therapy is required to aid rehabilitation and promote independence.
• Promote independence through the provision of assistive technology and signposting to community assets.
• Assess if long-term support is necessary and, where appropriate, create revised support plans to reflect changes in care needs.
• Complete Mental Capacity Assessments (MCAs) and risk assessments as needed.
• Make referrals to extra care housing and in-house provider services, as required.
• Ensure the highest standards of professional practice and safeguarding in all assessments and interventions.

Requirements:

• A recognized qualification in Social Work (Degree in Social Work, CQSW, CSS, or equivalent).
• HCPC (Health and Care Professions Council) registration.
• Proven experience in adult social care, particularly in hospital discharge, assessments, and care planning.
• Strong understanding of mental capacity legislation and safeguarding processes.
• Ability to work effectively as part of a multi-disciplinary team.
• Excellent communication and organizational skills.
