About the Role:

We are currently recruiting for an experienced Rough Sleep Support Worker to join North Northamptonshire Council’s Rough Sleeping Service.

This is an excellent opportunity for an experienced support professional to make a direct difference to people experiencing rough sleeping, homelessness and complex needs.

You will provide street outreach and housing-related support, helping individuals move from rough sleeping towards safe accommodation, stability and independent living.

Key Responsibilities:

• Deliver proactive street outreach across North Northamptonshire.
• Engage with rough sleepers and build trusting, professional relationships.
• Complete risk assessments and support plans.
• Support individuals to access accommodation, healthcare, benefits, substance misuse and other specialist services.
• Provide housing-related support to people in temporary accommodation.
• Help individuals develop independent living skills and sustain accommodation.
• Support move-on into longer-term housing and prevent repeat homelessness.
• Identify and respond appropriately to safeguarding concerns.
• Work closely with Housing, Adult Social Care, Health, Police, Hospitals, Prisons, landlords and voluntary sector partners.
• Support multi-agency meetings and crisis interventions where required.
• Maintain accurate case records and advocate for service users.

Requirements:

• Relevant experience supporting vulnerable adults.
• Experience working with homelessness, rough sleeping, housing support or complex needs.
• Experience of safeguarding, assessment and support planning.
• Knowledge or experience of mental health and/or substance misuse support.
• Strong communication and partnership-working skills.
• Ability to work independently within the community and engage individuals who may be difficult to reach.
• Full UK driving licence.
• Access to a vehicle.
• Enhanced DBS.
• Availability to work on-site 5 days per week.
