About the Role:
We are currently recruiting Reablement at Home Support Workers to join an award-winning organisation in Sunderland on a temporary contract. This is a rewarding, community-based role supporting individuals in their own homes following a decline in health, helping them regain independence, rebuild confidence, and improve their quality of life.

You will work directly with service users in the community, supporting their recovery journey and enabling them to live as independently as possible.

Key Responsibilities:
• Support individuals to relearn daily living skills
• Promote confidence, independence, and wellbeing
• Assist service users to achieve personal recovery goals
• Provide practical and emotional support in line with care plans
• Work flexibly across weekday and weekend shifts

Shift Patterns:
• AM: 07:00 – 15:30
• PM: 13:30 – 22:00
• Weekday and weekend shifts available

Requirements:
• Caring, positive, and enabling approach
• Strong communication and basic written skills
• Flexible, adaptable, and resilient attitude
• Ability to work evenings, weekends, and bank holidays
• Full UK driving licence (essential)
• Two satisfactory references
• Current DBS disclosure
• Right to work in the UK (no restrictions)
