About the Role:
Supply Care Solutions are currently recruiting for an experienced Occupational Therapist (Housing) to join Caerphilly County Borough Council. This is an exciting opportunity to provide specialist OT advice and support across social care, housing, and health services, with a strong focus on housing needs for disabled residents.

You will work closely with housing teams, social landlords, health professionals, and partner agencies to maximise the use of accessible and adapted housing across the County Borough, while supporting complex cases requiring specialist housing adaptations.

Key Responsibilities:
• Provide specialist OT advice and support to Caerphilly Homes teams and partner social landlords
• Undertake OT assessments for housing applicants referred via the Common Housing Register with medical needs
• Assess residents’ needs and prepare professional recommendations for housing adaptations
• Liaise with tenants, carers, health and social care professionals, contractors, and elected members
• Support Public Sector Housing contracts, including the Post Asset Management Scheme (PAMS)
• Assist with complex hospital discharge cases where significant housing needs are identified
• Contribute to Housing Strategy by identifying demand for accessible housing
• Provide specialist advice on adaptations and equipment for prospective and existing tenants
• Maintain up-to-date knowledge of housing registers, allocations policies, and accessible housing requirements

Requirements:
• Qualified Occupational Therapist with current HCPC registration
• Experience within social care, housing, or community-based OT services
• Ability to carry out site visits and assess housing adaptations
• Strong assessment, interpersonal, and report-writing skills
• Ability to work independently and manage competing priorities
