About the Role:
Peterborough City Council is seeking an experienced Occupational Therapist to join their Adult Social Care team in a highly rewarding, community-based role.

You will deliver a specialist service to adults and older people with complex and often fluctuating needs, including physical disabilities, long-term conditions, mental health challenges, learning disabilities, and age-related frailty. This role requires a high level of autonomy, advanced clinical reasoning, and the ability to manage complex caseloads involving health, housing, and social care factors.

Key Responsibilities:
• Independently manage a complex caseload within a community setting
• Undertake comprehensive functional assessments in service users’ homes
• Assess risk, functional ability, and need for equipment or adaptations
• Recommend and facilitate minor and major adaptations, including Disabled Facilities Grants
• Provide specialist advice on moving and handling and long-term support planning
• Apply relevant legislation including the Care Act 2014 and housing regulations
• Work collaboratively with multidisciplinary teams, families, and external partners
• Maintain accurate and compliant case records in line with statutory requirements
• Respond to urgent or duty cases where required, including safeguarding concerns

Requirements:
• Qualified Occupational Therapist with HCPC registration
• Proven experience within Adult Social Care or community-based settings
• Strong background in complex functional assessments and case management
• Knowledge of the Care Act 2014, housing legislation, and safeguarding frameworks
• Experience with equipment provision, adaptations, and moving & handling
• Ability to work independently and manage competing priorities
• Strong communication and multidisciplinary working skills
