About the Role:
Peterborough City Council is seeking an experienced Occupational Therapist to join their Adult Social Care team in a highly rewarding, community-based role.
You will deliver a specialist service to adults and older people with complex and often fluctuating needs, including physical disabilities, long-term conditions, mental health challenges, learning disabilities, and age-related frailty. This role requires a high level of autonomy, advanced clinical reasoning, and the ability to manage complex caseloads involving health, housing, and social care factors.
Key Responsibilities:
β’ Independently manage a complex caseload within a community setting
β’ Undertake comprehensive functional assessments in service usersβ homes
β’ Assess risk, functional ability, and need for equipment or adaptations
β’ Recommend and facilitate minor and major adaptations, including Disabled Facilities Grants
β’ Provide specialist advice on moving and handling and long-term support planning
β’ Apply relevant legislation including the Care Act 2014 and housing regulations
β’ Work collaboratively with multidisciplinary teams, families, and external partners
β’ Maintain accurate and compliant case records in line with statutory requirements
β’ Respond to urgent or duty cases where required, including safeguarding concerns
Requirements:
β’ Qualified Occupational Therapist with HCPC registration
β’ Proven experience within Adult Social Care or community-based settings
β’ Strong background in complex functional assessments and case management
β’ Knowledge of the Care Act 2014, housing legislation, and safeguarding frameworks
β’ Experience with equipment provision, adaptations, and moving & handling
β’ Ability to work independently and manage competing priorities
β’ Strong communication and multidisciplinary working skills
