TEAM: IDT Discharge Team
HOURS: Monday to Friday 08:45am – 5pm 37 hours per week
Team Managers will need experience of managing social workers in community or Hospital
Provide professional management, leadership and mentoring support to qualified Social Workers, Assistant Practitioners, Care Navigators (C/N) who form part of an integrated Multi-disciplinary team.
Responsible for grievance, staff performance, sickness monitoring, counselling and disciplinary matters- Responsible for staff probationary Reviews. Provide regular Monthly Supervisions to Professional Leads & Social Workers. Chair MDT & Operational meetings. Monitor Team Workload, Maintain Patient flow within the service Quality assure the assessments/support plans/reports completed to maintain High Levels of Professional standards and practice are adhered too in accordance with the Professional Capabilities Framework. Attend Daily Delayed Discharge of Care (LOS) meetings. Attend Stranded patient Reviews/Length of Stay Meetings-Identifying Effective & Efficient use of Resources, Budgets, Staffing.
Qualified Social Worker Requirements
– CSW/CQSW/DipSW/BA/MA in Social Work
– Proven post qualifying experience in working with Adult services
– Current Social Worker England registration
– Current Enhanced DBS Disclosure
– Eligibility to work in the UK without work permit or visa restrictions
– Commitment to the safe care and well-being of vulnerable people
