Job Description

This is an experienced practitioner role, expected to undertake cases independently and demonstrating an ability to adapt the approach from first principles to deal with more complex or unusual case, assessing and managing risks appropriately. The role is for Hospital Social Worker. We are looking for someone with hospital Social Work experience, able to assess and facilitate safe discharges in a timely manner. This is a fast-paced team working collaboratively with health partners, housing, and other stakeholders to facilitate hospital discharges from acute settings. The management and team are very supportive. The team employ hybrid work with some days working from home depending on the needs of the service. The candidate should be able to work with children (see the adult, see the children) and young people, families, carers, and communities to help them express their needs and make informed decisions and contribute to service planning. Note to Suppliers: Maximum pay rate will be in line with the London Borough Agreement

Qualified Social Worker Requirements

– CSW/CQSW/DipSW/BA/MA in Social Work
– Current Social Worker England registration
– Current Enhanced DBS Disclosure
– Eligibility to work in the UK without work permit or visa restrictions
– Commitment to the safe care and well-being of vulnerable people
