PAY RATE: £29.00 per hour (umbrella)
TYPE: Locum RMN
HOURS: Mon-Fri (09.00-17.00) = 37.5 hours per weel

Job Description:

  • The Care Coordinator will work as a member of the Community Mental Team for Older People (CMHT). The post holder will manage a defined caseload of service users living in the community who have a range of complex mental health needs.
  • They will undertake holistic community assessments of service users referred to using Risk Assessment tools, Mental State examination, and integrated clinical notes system.
  • To ensure the mental health needs of older people referred to the CMHT are appropriately assessed and a high level of quality care is delivered within available resources.
  • To provide support and advice to users, carers, and other professionals on mental health issues relating to older people.
  • To manage and have a Care-coordinator responsibility for a defined clinical caseload of service users with complex mental health problems living in the community. This will include service users subject to the Care Programme Approach and under Lead Professional care.
  • To undertake holistic assessments of service users utilising the integrated notes system, which will incorporate Risk Assessment and Mental State examinations.
  • To promote the work of the CMHT and an understanding of mental health issues affecting older people within the catchment area.
  • To participate in the on-call duty service for the CMHT and maintain up-to-date clinical records and documentation (including electronic records) in line with agreed local policies and procedures, NMC guidelines and CNWL policy.
  • To implement and maintain regular clinical and managerial supervision of junior staff working within the CMHT.
  • To ensure that own professional practice and clinical knowledge is maintained at all times in accordance with guidelines from relevant professional bodies (i.e NMC, HCPC) and CNWL policies and procedures.
  • To work in partnership with the primary care team, and statutory and voluntary agencies to ensure that the appropriate care needs of older people with mental health problems are met
