We are looking to cover a placement for a Band 6 Learning Disability Nurse for Community Specialist Health in Essex, starting ASAP for 20hrs a weeks for 12 weeks, between Mon-Fri 9-5.
PAY RATE: £27.00 per hour (umbrella)

Registered learning disability nurse. Need to be a car driver with access to a car.
You will work as part of a multi disciplinary team providing support to those people with learning disability with complex physical health need; challenging behaviour and; complex mental health. The role is support these people to remain in the community accessing appropriate health care. This also includes uptake of annual health checks; support of end of life planning and review of DNACPR; supporting with frailty. The person will need good networking and communication skills and prepared to work autonomously. This is a community based role with 4 bases to drop down, but the service covers all South Essex.
