About the Role:
An exciting opportunity has arisen for Hospital Social Workers to join the Transition Team at Stoke-on-Trent City Council. This full-time role (37 hours per week) focuses on supporting individuals transitioning from hospital to community settings, ensuring continuity of care and positive outcomes.

Key Responsibilities:
• Facilitate smooth transitions from hospital to home or alternative care settings
• Undertake assessments and develop care and support plans
• Liaise with healthcare professionals, families, and external agencies
• Promote independence and wellbeing for service users
• Maintain accurate documentation and case management records

Requirements:
• Qualified Social Worker with experience in hospital discharge or transitions
• Strong understanding of adult social care legislation and frameworks
• Up-to-date CV with no unexplained gaps
• Right to work in the UK (Passport, Visa, or Share Code)
• National Insurance Number
• Valid DBS certificate
