About the Role:
We are currently supporting a client who is seeking two experienced Band 6 Community Mental Health Nurses to join their Dementia Assessment & Diagnosis Team based in Telford and Shrewsbury. This is an initial 12-week locum placement, offering an excellent opportunity to work within a specialist community service supporting individuals with dementia and older adults’ mental health needs.

Key Responsibilities:
• Undertake specialist mental health and dementia assessments in the community
• Provide triage and urgent community assessments
• Manage and coordinate a complex caseload
• Develop care plans and carry out risk assessments
• Deliver evidence-based pharmacological and psychologically informed interventions
• Work closely with service users, carers, and families
• Collaborate effectively with multidisciplinary and multi-agency teams
• Support and supervise junior staff where appropriate
• Ensure compliance with NMC standards, Trust policies, and safeguarding procedures

Requirements:
• Registered Mental Health Nurse (NMC registered)
• Band 6 experience (or equivalent)
• Essential experience in dementia care or older adults’ mental health
• Full UK driving licence and access to a vehicle
• Ability to work full time (37.5 hours per week, 09:00–17:00)
