Hourly Rate: £35 per hour
Location: Halton Council – HICaF Discharge Team, Whiston Hospital

Key Responsibilities:

• Collaborating to ensure safe and efficient hospital discharges
• Managing a complex caseload independently and efficiently
• Providing support and intervention for individuals and families during their transition from hospital to community-based care
• Liaising with multidisciplinary teams to ensure seamless, person-centered care

About You:

• Previous experience working in an Integrated Hospital Discharge Team
• Knowledge of Eclipse (desirable)
• Ability to hit the ground running in a fast-paced environment
• A full driving license and access to a vehicle (essential)
