About the Role:
An exciting opportunity has arisen for a motivated and experienced Social Worker to join our Intermediate Care Team in Warrington. This part-time role (27 hours per week) is perfect for professionals passionate about supporting adults to regain independence, reduce hospital admissions, and facilitate safe discharges from hospital settings.

As an Adults Social Worker in Intermediate Care, you will work as part of a multidisciplinary team, conducting strengths-based assessments, developing support plans, and ensuring timely interventions that promote recovery and independence.

Key Responsibilities:
• Undertake Care Act assessments and support planning in line with legislation and local policies.
• Support safe hospital discharges and prevent unnecessary admissions.
• Work collaboratively with health colleagues in a fast-paced intermediate care setting.
• Advocate for service users and ensure safeguarding responsibilities are upheld.
• Maintain accurate and timely case records and contribute to performance monitoring.

Requirements:
• Qualified Social Worker with Social Work England registration.
• Relevant experience in adult social care, ideally in intermediate care or hospital discharge.
• Strong understanding of the Care Act and safeguarding responsibilities.
• Ability to work effectively in a multidisciplinary team.
• Excellent communication and record-keeping skills.
