About the Role:
We’re currently seeking a dedicated and experienced Adults Social Worker to join a supportive hospital discharge team in Darlington. This role focuses on enabling safe and effective hospital discharges using a Home First, strengths-based approach. You will review individuals placed in temporary residential placements following discharge and support their transition with appropriate care planning.

Key Responsibilities:
• Manage hospital discharge cases with a focus on recovery and independence
• Conduct assessments and reviews in temporary residential settings
• Develop and implement person-centred support plans
• Apply Care Act principles and local authority policies
• Liaise with hospitals, care providers, and community services to coordinate timely discharges
• Maintain accurate and up-to-date case records
• Participate in team meetings, supervision, and continued professional development

Requirements:
• Qualified Social Worker with Social Work England registration
• Minimum of 2 years’ experience in adult social care, preferably hospital discharge
• Up-to-date DBS certificate
• Eligibility to work in the UK (e.g., Passport or Biometric Visa)
• National Insurance number
• Strong understanding of hospital discharge protocols and strengths-based practice
