About the Role:
We’re recruiting for an experienced Adults Manager – Hospital Discharge to join the team at Stepping Hill Hospital, Stockport. This full-time, office-based role offers a competitive rate of Β£46 per hour and requires a valid driving licence to work flexibly across hospital and community bed-based sites.

As Adults Manager, you will lead a multidisciplinary team delivering safe, timely hospital discharges while maintaining strong partnerships with health services and other stakeholders.

Key Responsibilities:
β€’ Lead and manage a multidisciplinary team including qualified Social Workers, ASYE practitioners, apprentices, and unqualified staff.
β€’ Oversee and support safe, timely hospital discharges across all pathway work.
β€’ Maintain excellent working relationships with health partners, ICB colleagues, and third-sector organisations.
β€’ Provide supervision and management oversight to ensure high-quality, person-centred assessments and interventions.
β€’ Chair and attend safeguarding meetings, Court of Protection proceedings, and MDT meetings.
β€’ Ensure robust application of legislation including the Care Act, Mental Capacity Act, Best Interests Meetings (BIM), and safeguarding policies.
β€’ Promote a positive culture of performance, accountability, and support within the team.
β€’ Work flexibly across hospital and community bed-based sites as required.

Requirements:
β€’ Qualified Social Worker with Social Work England registration.
β€’ Proven experience in adult social care management, particularly in hospital discharge planning.
β€’ Valid driving licence.
β€’ Strong understanding of relevant legislation and safeguarding procedures.
β€’ Excellent leadership, communication, and partnership-working skills.
β€’ Enhanced DBS Certificate (must be registered on the Update Service).
