An exciting opportunity has arisen to join the Adult Social Services Team based at Norfolk and Norwich University Hospital. This role is ideal for a skilled social worker passionate about complex casework and multi-disciplinary collaboration.

You’ll be part of a fast-paced, supportive environment, working closely with a wide range of health professionals to support inpatients and facilitate safe hospital discharges.

👥 What You’ll Be Doing

• Carrying a caseload of complex discharge cases

• Completing Care Act Assessments and Discharge to Assess planning

• Conducting Mental Capacity Assessments

• Leading Safeguarding Enquiries

• Collaborating with a Multi-Disciplinary Team (MDT) on-site

• Ensuring timely and effective discharge planning

🧾 Requirements

• Social Work qualification (MA/BA Social Work, DipSW, or CQSW)

• Registration with Social Work England

• Experience working within hospital or fast-paced social work settings

• Confident in managing complex discharges and assessments

• Office-based role – must be able to attend in person

• Interviews to be held via video call
