About the Role:

We are excited to offer an excellent opportunity to join Halton Council’s Adult Social Work team as an Adult Social Worker within the Hospital Discharge Team, based at Whiston Hospital. This role is integral to supporting individuals during their transition from hospital to community care, ensuring safe, timely, and person-centred discharges.

Key Responsibilities:

• Ensure safe and efficient hospital discharges through collaborative, multidisciplinary work
• Manage a complex caseload, delivering timely and effective interventions
• Provide transitional support to individuals and families moving from hospital to community care
• Work alongside other professionals to deliver person-centred services that meet individual needs

Requirements:

• Qualified Social Worker with Social Work England registration
• Previous experience in an Integrated Hospital Discharge Team
• Knowledge of Eclipse (desirable)
• Valid UK driving licence and access to a vehicle (essential)
• Strong interpersonal, verbal, and written communication skills
• Ability to work effectively both independently and within a team
• Mentoring or supervision experience (advantageous)
• Commitment to ongoing professional development
