About the Role
We’re recruiting an Adult Social Work Assistant to join Nottinghamshire County Council’s Adult Services team. You’ll assess and review individual and carer needs, initiate and coordinate solutions to meet agreed personalised outcomes, and promote independence and personalisation so service users can manage their own needs with the right support.

Key Responsibilities
• Assess and review individual and carer needs, initiating and coordinating personalised interventions
• Identify reablement opportunities and provide access to those services
• Promote independence so individuals and carers can assess and manage their own needs, risks, and outcomes
• Identify community and other support resources, maximising individual assets, preventative services, and other funding sources
• Review ongoing service provision, ensuring alternative solutions are considered
• Provide information, advice, and support to service users and carers
• Apply the Mental Capacity Act practice framework as necessary
• Support Safeguarding Officers and Managers in enquiries (tasks within the assistant’s remit)
• Organise and manage your social care workload independently with appropriate oversight
• Undertake assessments using a range of health and social care tools (following appropriate training)
• Liaise and negotiate with local providers and support networks to deliver better outcomes
• Contribute to practice and service development, working flexibly as required
• Maintain confidentiality at all times and ensure information is only shared appropriately

Requirements
• A full driving licence and access to transport (reasonable adjustments available for disabled staff)
• Ability to meet agreed objectives and delivery targets by using resources flexibly and efficiently
• Compliance documents required:

• Up-to-date CV (with no unexplained gaps and date-specific employment history)

• Proof of Right to Work in the UK (passport, visa, biometric card, share code)

• National Insurance Number

• Valid DBS certificate
