Pay Rate: £27.40 ph umbrella depending on experience

Hours: Monday to Friday – Full time – 37 hours

Hybrid – 4 days working from home

Responsibilities:

Are you a passionate and dedicated social worker looking to make a difference in your community?

We are seeking a Social Worker to join our Locality Team, providing vital support and services to individuals with diverse needs.

Key Responsibilities: Conduct comprehensive assessments and develop care plans for individuals with physical and/or mental health needs. Implement and monitor interventions to support clients’ well-being. Work collaboratively with other professionals and agencies to provide care. Advocate for clients’ needs and rights within the community.

Ideal Candidate: Qualified and registered social worker with experience in adult social care. Strong assessment and care planning skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a team. The team undertake assessments face to face and work in a hybrid way, so we require someone who can be responsive in the local area and on site at least once a week.
