About the Role:
An exciting opportunity has arisen with Walsall Council in partnership with the NHS for experienced Physiotherapists and Occupational Therapists to join their Intermediate Care Service. There are 3 vacancies available across a mixed-discipline team.

You will work within a dynamic and integrated service delivering care across reablement, rapid response, and rehabilitation bed pathways. This role offers the opportunity to work in a fast-paced, multidisciplinary environment, supporting hospital discharge and preventing unnecessary admissions through effective community-based interventions.

Key Responsibilities:
• Deliver high-quality rehabilitation and reablement interventions in the community
• Work across Intermediate Care pathways including Rapid Response and Rehabilitation Beds
• Support discharge-to-assess models, facilitating safe and timely discharges
• Collaborate within a multidisciplinary team including health and social care professionals
• Undertake holistic assessments and develop person-centred care plans
• Maintain accurate clinical records and contribute to service improvement

Requirements:
• HCPC registered Physiotherapist or Occupational Therapist
• Post-qualified experience working within rehabilitation or Intermediate Care settings
• Community experience is essential
• Strong MDT working and communication skills
• Ability to manage a varied and complex caseload independently
• Full UK driving licence and access to a vehicle
