About the Role:
We are currently recruiting on behalf of Lincolnshire County Council for a Leaving Care Worker (G7) within Children’s Services. This role focuses on supporting young people aged 16–25 as they transition from care into independent adulthood. You will act as a key point of contact, coordinating support and ensuring young people receive the guidance and resources needed to achieve positive outcomes. The role offers a hybrid working pattern, with a minimum of 2 days in the office and the remaining time spent on visits or working from home.

Key Responsibilities:
• Act as the main point of contact for young people leaving care
• Coordinate and lead on pathway plans, ensuring access to appropriate services
• Provide direct support to young people, their families, and wider networks
• Work collaboratively with multi-agency partners to improve outcomes
• Support young people into education, training, employment, and stable accommodation
• Deliver targeted interventions for individuals at increased risk (e.g. anti-social behaviour, homelessness, poor mental health)
• Engage and build relationships with harder-to-reach young people
• Contribute to duty service, supporting young people in crisis

Requirements:
• Experience working with children, young people, or care leavers
• Strong knowledge of both Children’s and Adult Services
• Ability to manage complex cases and work independently
• Full UK driving licence and access to a vehicle
• Willingness to travel nationally and undertake overnight stays where required
