About the Role:
Stoke-on-Trent City Council is seeking experienced Adult Social Workers to join the NMH Harplands Hospital Team. This is a full-time opportunity (37 hours per week) working within a hospital setting, supporting individuals with mental health needs and ensuring safe, effective discharge planning and ongoing care coordination.

Key Responsibilities:
• Complete assessments and care planning for adults within a hospital setting
• Support safe and timely hospital discharges
• Work collaboratively with multidisciplinary teams including healthcare professionals
• Safeguard vulnerable adults and manage risk effectively
• Maintain accurate and up-to-date case records

Requirements:
• Qualified Social Worker with relevant Adult Social Care experience
• Experience within hospital or mental health settings preferred
• Up-to-date CV with full employment history and no unexplained gaps
• Right to work in the UK (Passport, Visa, or Share Code)
• National Insurance Number
• Valid DBS certificate
