💼 Hours: 37.5 per week
💰 Rate: Up to £27.00/hr weekdays

About the Role:
I’m currently working with an NHS client in Stevenage who is looking to recruit an experienced Band 6 Care Co-ordinator to join their Early Intervention in Psychosis (EIP / PATH) Service within Adult Community Mental Health Services. This is an excellent opportunity to work in a specialist multidisciplinary team, supporting individuals aged 16–65 experiencing or recovering from a first episode of psychosis, delivering recovery-focused care aligned with NICE guidelines.

Key Responsibilities:
• Manage a caseload of service users experiencing first episode psychosis
• Complete specialist mental health assessments and care planning
• Deliver recovery-focused and evidence-based interventions
• Work closely within a multidisciplinary team across health and social care
• Support service users through the 3-year Early Intervention pathway
• Deliver Family Interventions (training provided and supervised by a psychologist)

Requirements:
• Registered CPN (NMC), Occupational Therapist (HCPC), or Social Worker (Social Work England)
• Minimum 1 year experience in Community Mental Health
• Experience with care coordination and risk assessment
• Full UK driving licence and access to a car
• Ability to work autonomously in a community setting
