About the Role:
Supply Care Solutions is currently recruiting for an Outreach Family Support Worker to join Liverpool City Council within the Early Help service. This role offers an excellent opportunity to work directly with children, young people and families using a whole-family approach to provide targeted Early Help support, improve outcomes, and prevent escalation to statutory services.
The successful candidate will work within the Early Help Hub Practice Standards and align their practice with the Signs of Safety model, using outcome-focused approaches to support families in achieving sustainable positive change.
Key Responsibilities:
β’ Managing complex cases that step down from Childrenβs Social Care (Level 3)
β’ Completing Early Help Assessments and coordinating Team Around the Family (TAF) meetings
β’ Acting as Lead Professional to coordinate services and improve outcomes for children and families
β’ Providing intensive and targeted outreach support to vulnerable children and young people aged 0β25
β’ Supporting families in developing practical parenting strategies and improving family relationships
β’ Working with adolescents experiencing emotional and mental health challenges
β’ Using de-escalation techniques to manage challenging behaviour
β’ Maintaining accurate case records, assessments, plans and chronologies on electronic systems
β’ Visiting families in their homes to provide direct support and intervention
β’ Working collaboratively with multi-agency professionals to deliver coordinated support plans
β’ Promoting safeguarding, equality, and anti-discriminatory practice in all aspects of work
Requirements:
β’ Experience working with children, young people, and families within Early Help or similar services
β’ Ability to manage complex cases and deliver targeted family support
β’ Experience completing Early Help Assessments and coordinating multi-agency meetings
β’ Strong communication and partnership working skills
β’ Enhanced DBS registered on the Update Service
β’ Full right to work in the UK
