About the Role:
We are currently recruiting an experienced Occupational Therapist to join a dynamic team in Hackney. This is a full-time hybrid role offering the opportunity to support adults with permanent or complex disabilities, promoting independence and preventing the need for ongoing services wherever possible. You will work as part of a Multidisciplinary Team, carrying out assessments, reviewing care provision, and ensuring interventions are effective and person-centred.

Key Responsibilities:
• Conduct comprehensive community assessments for adults with permanent or complex disabilities
• Provide services and interventions that reduce reliance on long-term care
• Carry out assessments under relevant legislation, including the Care Act and Housing Grants, Construction and Regeneration Act
• Review existing care provision to support independence and minimise handoffs
• Supervise Occupational Therapy Assistants and OT students as required
• Contribute to tasks within the OT and Sensory service to support Adult Social Care ambitions
• Collaborate with service users, carers, and other professionals to maximise independence
• Review support packages, equipment, and adaptations
• Maintain accurate and timely case notes on appropriate systems at all times
