About the Role:
We are currently recruiting for an experienced Adult Social Worker to join a Mental Health Hospital Discharge Team based in Dudley. The role focuses on ensuring timely, safe, and person-centred discharges from hospital, with strong partnership working across health, social care, and voluntary sector organisations.

The successful candidate will play a key role in supporting service users and carers to achieve the shortest possible hospital stay, ensuring appropriate personalised care and support is in place to maintain independence in the community.

Key Responsibilities:
• Ensure service users and carers experience the shortest possible length of stay in institutional settings
• Facilitate efficient and effective discharge planning with personalised, flexible support packages
• Complete Care Act assessments and make appropriate recommendations
• Undertake Mental Capacity Assessments where required
• Attend ward reviews, pre-discharge meetings, and Section 117 reviews
• Work closely with housing providers and external agencies to support timely discharges
• Commit to regular meetings with health discharge coordinators
• Act as a key point of contact between health and social care, escalating issues where necessary
• Maintain effective communication with locality Adult Social Care teams
• Explore respite and step-down care options
• Attend funding panels, present cases, and make recommendations
• Support Section 117 funding tool applications
• Act as a bridge between organisations to promote effective joint working between health and social care

Requirements:
• Qualified Social Worker with relevant experience in adult mental health
• Registered with Social Work England
• Strong knowledge of Care Act and Mental Capacity legislation
• Experience of hospital discharge planning
• Ability to work collaboratively across multi-agency settings
