About the Role:
Supply Care Solutions is recruiting an experienced Community Occupational Therapist to join a dynamic community-based team in South East London. This role involves working closely with service users, carers, families, and multi-disciplinary professionals to promote independence, safety, and quality of life.

You will deliver high-quality occupational therapy services across a variety of settings, including assessments, rehabilitation planning, equipment provision, environmental adaptations, care planning, and case management.

Key Responsibilities:
• Meet with service users, carers, families, and multi-disciplinary agencies across community settings
• Carry out holistic assessments to promote independence and wellbeing
• Develop and implement individual care, support, and rehabilitation plans
• Specify equipment and recommend structural or environmental adaptations
• Coordinate assessments, reports, and presentations to relevant multi-agency panels
• Monitor progress and manage cases effectively
• Ensure all work complies with Council policies, procedures, and statutory duties
• Adhere to the College of Occupational Therapy Code of Ethics and HCPC Standards of Proficiency

Requirements:
• Qualified Occupational Therapist with community experience
• HCPC registration
• Ability to manage a varied caseload independently
• Strong communication and multi-disciplinary working skills
• Commitment to high professional standards and best practice
