About the Role:

Walsall Council is seeking an experienced Adult Social Worker to join their Intermediate Care Services team based at Walsall Manor Hospital. This is an excellent opportunity to work in a fast-paced, multidisciplinary environment supporting adults recovering from illness or injury and requiring short-term intervention.
You will carry out strengths-based assessments, risk assessments, care planning, and discharge planning while ensuring compliance with relevant legislation and local policies. Strong collaboration with health professionals, OTs, physiotherapists and wider community teams is essential for this role.

Key Responsibilities:

• Complete strengths-based assessments and risk assessments
• Develop personalised care plans
• Facilitate safe and timely hospital discharges
• Work closely with health and therapy teams within the hospital
• Make confident, evidence-based professional decisions within a busy environment
• Ensure compliance with statutory duties and local procedures

Requirements:

• Qualified Social Worker
• Social Work England registration
• Enhanced DBS on the Update Service
• Valid driving licence
• Previous experience in Adult Social Care—hospital or intermediate care experience preferred
