About the Role:
Supply Care Solutions is recruiting for an experienced and motivated Occupational Therapist Assistant to join Hackney Council’s Adult Social Care Reablement Service. This role is ideal for someone passionate about delivering strengths-based assessments, promoting independence, and supporting residents to live safely and confidently in their homes.

Key Responsibilities:
• Carry out strengths-based OT assessments and reviews in line with relevant legislation (Care Act, Housing Grants & Construction Act).
• Provide OT interventions including equipment provision, retraining, environmental recommendations, and minor/major adaptations (with OT support).
• Identify and manage risks related to the use of specialised equipment within the home.
• Complete comprehensive mobility assessments, including eligibility for Blue Badges, Freedom Passes, and Parking Bay requests.
• Work collaboratively with service users, carers, OTs, Social Workers, Senior Practitioners and external agencies.
• Provide accurate advice, signposting, and support towards TeleCare and universal services.
• Recognise vulnerable individuals and follow safeguarding pathways.
• Produce clear and accurate written and verbal reports, ensuring timely updates on case management systems.
• Contribute to a supportive and inclusive team environment, participating in meetings, training, and reflective practice.
• Manage and prioritise your caseload while maintaining high professional standards.

Requirements:
• Accredited Trusted Assessor Qualification.
• Enhanced DBS (must be on the Update Service).
• Strong clinical reasoning and decision-making abilities.
• Excellent IT literacy and accurate recording skills.
• Ability to work independently and collaboratively across multi-disciplinary teams.
• Understanding of strengths-based and person-centred practice.
• Knowledge of Care Act 2014 and OT-related legislation.
• Experience conducting OT assessments and supporting independence through adaptive interventions.
