About the Role:
We’re recruiting on behalf of our client in Sunderland for three experienced Care Coordinators to join a dedicated Supported Living Service that empowers individuals with complex needs. You will play a key role in promoting independence, enhancing quality of life, and ensuring high-quality care delivery across the service.
You will support customers with a wide range of complex needs, including Learning Disabilities, Autism, Behaviour that Challenges, and Mental Health needs, helping them to build skills, engage with their communities, and live fulfilling lives.

Key Responsibilities:
• Provide physical, emotional, and psychological support
• Deliver intimate personal care where required
• Support customers with a range of health needs
• Assist with practical day-to-day tasks
• Promote meaningful communication
• Enable community participation
• Work confidently within challenging environments using Positive Behavioural Support
• Support team members and work within a multi-agency framework

Requirements:
• Experience supporting individuals with complex needs
• Strong alignment with core organisational values: Dignity, Respect, Compassion, Integrity, Responsibility, and Teamwork
• Up-to-date accredited training such as Safe Handling of Medication, Moving & Assisting, First Aid (highly advantageous)
• Flexibility to work within set rota patterns
• Ability to work dynamically and remain motivated in challenging settings
