About the Role:
Weβre seeking a dedicated and experienced Adults Social Worker for a 6-month fixed-term project in Newcastle. This is a fantastic opportunity to apply your expertise in a vital area of social care, focusing specifically on Continuing Healthcare (CHC) case reviews.
As a Social Worker on this project, you will provide essential capacity to review CHC cases in line with statutory responsibilities.
Key Responsibilities:
β’ Plan and participate in Decision Support Tool (DST) and Multi-Disciplinary Team (MDT) meetings
β’ Manage disagreements and disputes during the review process
β’ Complete Care Act Assessments as required
β’ Prepare for DST meetings by gathering information from internal systems (e.g., Eclipse), providers, individuals, and families
β’ Attend DST meetings and accurately document outcomes
β’ Handle situations where CHC is not agreed, including formally disputing with the Integrated Care Board (ICB) in writing, attending reconvened DST meetings, and drafting dispute letters for management submission
β’ If CHC is not agreed and there is no dispute, complete Care Act Assessments, develop Support Plans, request financial assessments, and facilitate transfer of cases and funding to Newcastle City Council
β’ Provide clear, supportive follow-up communication with individuals and families about the process
Requirements:
β’ Qualified Social Worker with professional registration
β’ Proven experience in adult social care
β’ Strong understanding of Continuing Healthcare (CHC) processes and criteria
β’ Familiarity with the Care Act 2014 and statutory duties
β’ Excellent communication, negotiation, and report-writing skills
β’ Ability to manage complex cases and work effectively in a multi-disciplinary environment
β’ Proficiency in social care IT systems (experience with Eclipse desirable)
