💰 Rate: Up to £26.01 per hour (Umbrella)

About the Role:
We’re seeking a dedicated and experienced Adults Social Worker for a 6-month fixed-term project in Newcastle. This is a fantastic opportunity to apply your expertise in a vital area of social care, focusing specifically on Continuing Healthcare (CHC) case reviews.

As a Social Worker on this project, you will provide essential capacity to review CHC cases in line with statutory responsibilities.

Key Responsibilities:
• Plan and participate in Decision Support Tool (DST) and Multi-Disciplinary Team (MDT) meetings
• Manage disagreements and disputes during the review process
• Complete Care Act Assessments as required
• Prepare for DST meetings by gathering information from internal systems (e.g., Eclipse), providers, individuals, and families
• Attend DST meetings and accurately document outcomes
• Handle situations where CHC is not agreed, including formally disputing with the Integrated Care Board (ICB) in writing, attending reconvened DST meetings, and drafting dispute letters for management submission
• If CHC is not agreed and there is no dispute, complete Care Act Assessments, develop Support Plans, request financial assessments, and facilitate transfer of cases and funding to Newcastle City Council
• Provide clear, supportive follow-up communication with individuals and families about the process

Requirements:
• Qualified Social Worker with professional registration
• Proven experience in adult social care
• Strong understanding of Continuing Healthcare (CHC) processes and criteria
• Familiarity with the Care Act 2014 and statutory duties
• Excellent communication, negotiation, and report-writing skills
• Ability to manage complex cases and work effectively in a multi-disciplinary environment
• Proficiency in social care IT systems (experience with Eclipse desirable)
