About the Role:
Are you a highly skilled Social Worker looking for your next challenge in Portsmouth? We’re seeking a Qualified Social Worker (Higher Grade) to join dynamic Hospital and Community Teams, where your expertise will directly impact the lives of vulnerable adults and their carers. This role offers the chance to work on complex and rewarding cases in a supportive environment.

Key Responsibilities:
• Assess the needs of service users and develop creative, personalised support plans
• Manage complex caseloads across a variety of client groups, including Older People, Adults with Physical/Sensory Disabilities, and Adults with Learning Disabilities
• Operate within either the Community Team or Hospital Team setting
• Work independently while maintaining clear and accurate case records
• Take an active role in safeguarding adults and applying relevant policies and procedures
• Apply statutory legislation and guidance in your day-to-day decisions
• Contribute to service development, including involvement in project work

Requirements:
• Recognised Social Work qualification (DipSW, MSc, CQSW, CSS)
• Current registration with Social Work England (formerly HCPC)
• Substantial post-qualification experience
• Strong knowledge of Adult Social Care eligibility criteria
• Excellent assessment and safeguarding skills
• Full understanding of personalisation in social care
• Ability to manage a diverse caseload and maintain accurate records (including for court use)
• Strong communication, organisational, and conflict resolution skills
• Ability to work under pressure and handle changing priorities
• Previous supervisory experience desirable
• Confident with IT systems and digital record-keeping
