Type: Temp – 6 month agency contract with potential for ongoing extension

Hours: Full-time position (37 hours)

Pay Rate: Competitive rate of pay of up to £38.00 per hour depending on experience

About the Role:

To undertake scheduled reviews and monitoring visits to residential and nursing homes. To promote and enhance the lives of people living with dementia within a residential/nursing placement. To work with providers to improve the opportunities for people living with dementia within a placement setting. To provide reports to the Team Manager on visits to placement settings. To liaise with other professionals and out health colleagues.
