Salary: The umbrella rate will be £21.50 per hour.

Job Description: 

Band 5 nurse to complete assessment of patients, core skill of any Nurse, assess and collate medical nursing and social history from the admitted patient in bed based in the acute, complete ToC with info then this info is passed to B6’s in the assessment team for final completion of discussions and pathways etc. Needs to be RN to gather and understand the clinical rationale on pathway decision for discharge from the acute hospital.
