About the Role:
Halton Borough Council is seeking a committed and compassionate Qualified Social Worker to join their Intermediate & Frailty Team. This is an excellent opportunity to support some of the most vulnerable residents, helping them maintain independence, improve well-being, and manage complex health and social care needs safely and effectively.

Key Responsibilities:
• Work closely with older adults and people with frailty and complex needs
• Carry out strengths-based assessments and develop person-centred care and support plans
• Collaborate with multi-disciplinary partners including hospital teams, community services and voluntary organisations
• Support safe and timely hospital discharges and help prevent avoidable admissions
• Undertake Mental Capacity Act assessments and safeguarding enquiries as required
• Promote independence, resilience and improved outcomes for adults and their families

Requirements:
• Qualified Social Worker with Social Work England registration
• Experience within adult social care—preferably frailty, hospital discharge or intermediate care
• Strong risk assessment and safeguarding experience
• Able to work confidently in fast-paced, integrated environments
• Passionate about delivering high-quality, person-centred care
